Sharing the Learning from the Evercare Project

Thursday 13th November, Birmingham: a joint event by NatPaCT, the DOH and United Health Group, bringing together project managers, lead clinicians and others from the nine PCTs implementing the Evercare model.

Morning session

Aims of the day Roy Paterson NatPaCT

The aims of the day were to better understand the issues arising from the first phase of implementation, share solutions to emerging problems and capture learning for other organisations. The day commenced with a series of questions about the Evercare model and its implementation.

Taking stock Facilitated discussion

Initially delegates were asked to consider what had been the three best things to come out of their local projects and the three biggest challenges to emerge from the work.

Considering the challenges, delegates were asked to discuss how they had tried to tackle barriers; to what extent they had succeeded in overcoming them; what resources they had used or developed to help and what had been their key learning in tackling these barriers?

Successes and Challenges Facilitated discussion

The conference then brainstormed the three most important things that UHG had brought to the project. The focus of the discussion then shifted to thinking about where UHG added the most value and how this could be done if Evercare were not involved.

Afternoon Session

Establishing Interfaces with Secondary Care, Trish Skitt, Walsall

Capturing Learning, Facilitated discussion

Where PCTs would be starting from scratch now – what they would want on the NatPaCT website to support them. These could be split into the following areas of: Stories; Q & A; Tools; Contacts; Model; Project Planning; Business Case.

Sharing Resources, Facilitated discussion

When considering how other PCTs could be successful without UHGs involvement – what resources have PCTs developed that would help other PCTs to implement Evercare and what other resources could be developed to help other PCTs implement this model.

Close - the most important things for PCTs to take away from the event

The event concluded by PCTs identifying practical solutions and lessons from the day’s exchanges. These are highlighted for the different sites below.

 Bristol
  • Look for a more creative use of the limited budget
  • Explore the concept of ‘Homecare’ on prescription.
  • Explore creating a register of vulnerable elderly and the services they receive. 

North Tees

  • To create an opportunity to present their project to the other PCTs
  • To reinforce the learning from the Evercare project to the elderly care in their area.

Luton

  • To explore use of existing queries for accessing primary care data
  • Explore use of IT for ‘text message’ alerts to APN mobile phones.
Halton

  • To employ the programme and clinical leads out in the practices more to reinforce the model and overcome resistance.

Airedale

  • To review the project plan and to redraw it for the next stage of recruitment

Bexley

  • To draw up a data action plan
  • Examine the flagging of caseload within the system.

Walsall

  • To strengthen the financial reference costs around the project – being mindful about what they say they’ll deliver
  • Utilising resources to share communication.
