The NHS and Social Care Long Term Conditions model
Three national events are planned on 8, 9 and 11 February in Bolton, Leicester and London, supporting the NHS and Social Care Long Term Conditions model.

Using data to Drive Health System Performance
A training programme to deliver this learning module is running in February and March 2005.  The purpose of the events will be to enable local health economies, lead by PCTs, to become better at extracting and analysing data to influence planning and commissioning.  The module will also include better use of population management approaches (segmentation) and risk stratification components which will support PCTs in the delivery of management of patients with Long Term Conditions.

Making the Connections - Managing Long Term Conditions & Patient and Public Involvement
Engaging Communities Learning Network run workshops to explore the interconnections between engaging communities/ patient and public involvement and managing long term conditions, including co-designing services and whole system working practices.
